Prior to any treatment:

You are required to have a consultation with me.

You are required to provide your details (Name, DOB, Address, Mobile Number, Email) necessary for me to issue a prescription of the product.

You are required to sign a consent form.

After any treatment:

I am available 24/7 to be contacted. I may not reply straight away due to my clinical commitments, but I will reply shortly. In case of any complication that occurred, I would like to be contacted so we can manage it promptly.

Treatment specifics:

You will need to pay for any treatment top up (including BoTX).

The payments are non-refundable, even in the event of complication occurrence.

I manage my own complications for free. However, for the complications occuring elsewhere, I will ask for payment.

General:

I require 24h notice for any cancellations. If it is less than 24h notice, I will take 15% of the total cost unless the reason is valid.

I don’t treat people below the age of 18.
